Please mention at the time of booking if you have any medical conditions, which may affect your treatment. Clients will be required to fill out a consultation form and answer questions about their health in order to determine suitability to treatment e.g. if you are likely to experience any allergic reactions or if there is a medical condition which could be viewed as a contraindication to treatment. Depending on the answers, therapy may be refused or a request may be made for either a GP's letter of authorisation or written consent by the client or main carer. However, every effort will be made to offer an alternative, more suitable treatment.
Or I may ask you to sign a disclaimer form before the treatment is carried out. I reserve the right to refuse to carry out a treatment which I feel is not suitable.

Equilibre take every possible care to ensure the safety and minimise risk to all party guests, use non-allergenic, high quality products and stringent hygiene precautions. Liability/responsibility cannot be taken for reactions, contra-indications or injury via activities partaken at or issue with any part of the service - sustained whilst at a party or thereafter, with a view to treatments, application/removal of products applied and food/drink consumed. All allergies, ailments, infestations and viruses of guests and clients, which may affect the party in any way, must be reported prior/on the party date, for discussion and prior agreement. We cannot accept liability for events outside the company's control to include hazards and dangers within your home or chosen venue.
